I would like to work with you to develop your wishes for your end of life care. It would be my privilege to help you, by sharing the “Story of You”.
While providing end of life care, I will work directly with you and your family to bring your wishes to fruition. While best laid plans can go awry I will strive to ensure that what you want is respected and honored.
I will also advocate for you with your family or your medical team to bring your wishes to the forefront.
Dying should be no different than being born, there should be a celebration, your life and all that you have accomplished should be remembered.
All everyone wants is to be is remembered.
